Learning Collaborative

Region 3's Learning Collaborative is an adaptation of the Institute for Healthcare Improvement's (IHI) Breakthrough Series. Through this model, participating providers develop specific shared goals for healthcare improvement at the organizational and/or regional levels. Intended to be achieved in a short time frame (6 - 12 months), results are then shared throughout the region. Cohort workgroups, are one of several opportunities available to regional stakeholders within the Learning Collaborative structure. Other opportunities for learning include monthly region-wide status and information calls, on the spot learning for urgent needs, peer-to-peer support and external learning and training announcements. Additionally, the Learning Collaborative provides a venue for the Region 3 to share celebrations of success. This is accomplished through region-wide communications, project spotlights in newsletters and the website, and a project fairs at large Learning Collaborative conferences held twice a year. These large Learning Collaborative conferences are core to the Region 3 Learning Collaborative structure in that they are culminating events where important learning information is shared.

All components of the Learning Collaborative are supported by a stakeholder driven structure. This includes the Learning Collaborative Steering Committee which provides guidance on the large conferences, measuring the impact of the Learning Collaborative and overall marketing of learning opportunities. Additionally, two advisory groups provide guidance to the cohort workgroups throughout their work. The Quality Improvement and Data Advisory Groups are described below and include anchor staff, provider volunteers and support from the University of Texas School of Public Health (UTSPH).

Most importantly, Learning Collaborative activities are not reserved for those providers directly participating in the 1115 Waiver. All stakeholders in Region 3 are encouraged to participate. To learn how to get involved, email setexasrhp@harrishealth.org.

Data Advisory Group
The Data Advisory Group is composed of members within the Region 3 that have experience and expertise in data management, assessment and evaluation. The primary role of this group is to work closely with Learning Collaborative cohort workgroups and the Quality Improvement Advisory Group to identify and facilitate areas for learning and data analysis, while assessing success through data captured for metrics defined by the individual cohort workgroups. Additionally, this group supports the broader region by assisting with analyzing available data to monitor the impact of the 1115 Waiver throughout the nine county region.

Quality Advisory Group
The Quality Improvement (QI) Advisory Group is composed of members within Region 3 that have a broad skill mix in facilitation and quality improvement. The primary role of this group is to work closely with Learning Collaborative cohort workgroups (as developed) and the Data Advisory Group to identify and facilitate areas for learning and continuous quality improvement while assessing success through data captured for metrics defined by the individual cohort workgroups.

Both advisory groups are also supported through a relationship with UTSPH. Faculty and graduate assistants support Data Advisory Group members through research, data analysis and best practices information. UTSPH supports members of the Quality Advisory Group through the implementation of the IHI Model and Plan-Do-Study-Act (PDSA) cycles.

Cohorts
Cohort workgroups (cohorts) were developed through the Region 3 Learning Collaborative to cluster providers with similar goals and projects for focused Continuous Quality Improvement (CQI) activities. These cohorts were developed based on analysis of survey and project data by the Region 3 Learning Collaborative Data and Quality Improvement Advisory Groups. Learning Collaborative participants self-select into appropriate cohort(s) based on interest and applicable projects. Cohort meetings will serve as a learning environment for all participants as members share data and experiences for CQI opportunities throughout Region 3 and the State of Texas. The Learning Collaborative Data and Quality Improvement Advisory Groups will support these cohorts with needed resources, including but not limited to: data, analysis of data, facilitation, subject matter expertise and improvement methodology coordination. Cohorts will be responsible for presenting data, results, and learned lessons at semi-annual regional Learning Collaborative meetings.
