Nursing Care Delivery Structure
The Nursing Care Delivery structure considers the nurse-patient assignment structures, criteria-based assignment process, and information management tools. Assignment structures are based on the AACN’s Synergy Model’s patient profiles and nurse competencies. Nursing care delivery models are selected by inpatient and ambulatory settings based on the patient profiles and complexity of care requirements.

Patient Information and Management Systems
Information management tools developed within in the electronic health record environment include best practice alerts and evidence-based, standard congruent care plan templates for commonly seen patients, which may individualized and patient-centered.

Nursing Quality and Outcomes Management
Nursing quality and outcome structures lay the foundation for an effective care delivery system. Dedicated to quality patient care and outcomes, Harris Health Nursing participates in the National Database on Nursing Quality Indicators (NDNQI), which assists staff in measuring nursing-sensitive indicators and comparing outcomes to other facilities across the United States.
