New Patient Information

To help ensure that your visit with us goes smoothly, we have outlined some brief tips below.

If you are a new patient to the Harris Health and do not have insurance, you will have to first establish eligibility for medical assistance at one of our eligibility centers.

For your first Harris Health visit, please note the following:

You should arrive at least 30 minutes before your appointment time so that you can be signed in by our registration staff. You will need a picture ID such as a driver’s license.

If appropriate to your visit, please bring any available X-rays or scans from outside physicians.

Please bring a list of all medications you currently are taking or have taken recently, whether over-the-counter or prescription. Include the strength and dosage of each medication.

Please bring your medical records from your previous physician.

If you are visiting us for an outpatient doctor appointment, please bring a list of personal health questions or concerns. Sometimes we forget all the things we want to tell or ask the doctor, so make a list of your symptoms, questions or concerns before you come to your appointment. And don’t forget to bring it with you!
