CHW Home Visit Program

Harris Health’s Community Health Worker Home Visit Program embodies Harris Health System's health hub philosophy and takes it on the go. This program brings tailored diabetes education directly to the homes of patients who have uncontrolled diabetes and are disengaged from care. Patients are educated on how to effectively navigate Harris Health to increase their connection to their diabetes care team.

Community health workers also assess for and address health-related social needs that may be getting in the patient’s way of achieving a healthier life. They have extensive knowledge and direct connections to a variety of community resources that can assist patients with anything from food to transportation.

In addition, the community health worker serves as a direct connection to patients’ medical homes. The goal of the program is to avoid long-term complications by improving chronic disease self-management through continued education, patient empowerment, and maximizing the care team’s resources.
