Healthy Heart, Healthy You

​The Healthy Heart, Healthy You Program is a care coordination home blood pressure monitoring program supported by licensed vocational nurse patient care coordinators and an operations coordinator in conjunction with the patient’s primary care provider.

Each enrolled patient receives a blood pressure monitor free of charge that is theirs to keep. Our primary goal is to improve blood pressure management, navigation to appropriate medical and community services, and just-in-time education regarding self-management of high blood pressure. Evidence-based strategies are utilized to improve blood pressure and reduce risk of hypertension related complications in alignment with our quality measures.

The program is currently active at Martin Luther King (MLK) Jr. Health Center, with future plans to expand to additional health centers. Hours of operation are Monday to Friday, 8 a.m.-7 p.m., with weekend availability as needed.

The program is currently receiving referrals for MLK Health Center patients that meet the criteria below:

Inclusion criteria

  • Diagnosis of hypertension
  • 18-85 years old
  • Bluetooth enabled smartphone or tablet
  • Active MyHealth account
  • Commitment to participate in regularly scheduled coaching calls, video and home visits
