How to Get a Copy of Your Medical Records
You can ask for your medical records in these ways:
Option 1: Fill Out a Form
Option 2: Written Request
You may also send a written request. Please include:
- Your full name
- Date of birth
- Address and phone number
- What records do you need (dates or types of visits)
- Who the records should be sent to
- Why you need the records
- Your signature
- A copy of your photo ID
Option 3: Request in Person
- Visit any Harris Health facility
- Fill out an authorization form or provide a written request on-site
- Bring a photo ID
Giving clear and complete information helps us send your records within 15 business days.

Get Records Online with MyHealth
