Patient Centered Medical Home

Harris Health makes patient care the focus of all healthcare providers. 

The patient-centered medical home (PCMH) is a model of care that fosters better relationships between patients and their clinical care teams.

Research shows that PCMHs improve quality and the patient experience, and increase staff satisfaction—while reducing healthcare costs. Practices that earn recognition have made a commitment to continuous quality improvement and a patient-centered approach to care.

What is a medical home? 
A medical home is the health center where patients receive check-ups, annual physical exams, screenings, immunizations, follow-ups for health conditions or for those feeling sick. Seeing a doctor on a regular basis can keep patients healthy and ensure they receive timely and appropriate treatment. 

How do I start my care with a provider? 
Call 713-526-4243 to make an appointment with a Harris Health provider at a health center near your home or a location that’s convenient for you. Your first visit will address your health concern(s) and allow the provider to learn about your medical history. This will become your medical home. You can always change your medical home location by talking to staff at the clinic’s front desk. 

Can I see specialists at Harris Health? 
Yes, we have renowned experts in a variety of specialties available to you for many conditions including diabetes, cancer and heart disease. Your primary care doctor will refer you to specialists if they consider it necessary to manage your health condition. 

What do I do after hours if I am not feeling well? 
If you are experiencing symptoms or need medical attention and it’s after hours, call Ask My Nurse, the nursing hotline (713-634-1110), available 24 hours a day, seven days a week, to offer advice on possible self-care at home, a clinic visit or care at a hospital emergency center. In an emergency, call 9-1-1.  

Same-day care is a good option for minor illnesses and injuries that are not life threatening. Harris Health has Same-Day Clinics that are open 8 a.m. to 8 p.m. Monday through Saturday (closed 1-2 p.m.). No appointment is necessary. What if I have an emergency and need immediate help? 
Call 9-1-1. If dealing with suicide intentions, call the Suicide Hotline at 988. 

​How do I stay connected to Harris Health? 
Once established as a patient, you can enroll in the MyHealth patient portal. It allows you to make appointments, refill prescriptions, view test results and send messages to your doctor 24 hours a day, seven days a week. Additionally, you can schedule a same-day Video Visit Now, a virtual visit with a doctor, from 8 a.m. to 8 p.m. Monday through Saturday.
