Harris Health's House Call Service is a program in collaboration with medical partners from The University of Texas Health Science Center at Houston (UTHealth) and Baylor College of Medicine that provides patients with high-quality, compassionate primary medical care and palliative care in the comfort of their home.
The team of dedicated board-certified physicians, nurse practitioners, registered nurses, licensed vocational nurses, case managers and social workers is sensitive to the special healthcare needs of patients.
The House Call Service provides the following services and is available in person, by telephone or virtually 24 hours a day, seven days a week:
- In-home primary care for management of medical problems
- In-home palliative care to improve quality of life
- Telehealth visits
- Transition of care visits/post hospitalization discharge follow up
- Home safety evaluations
- Care coordination with your care team for needs support services
- Preventative care management including health assessments and immunizations
Our Team
House Call Service focuses on direct patient care by fostering strong collaboration with patients, families and their caregivers. By providing compassionate and comprehensive care, our providers in partnership with the case managers are able to deliver solid initiatives to meet individual care goals.
House Call Service has two teams (green and orange) that serve Harris County residents. The green team supports geriatric patients, while the orange team supports palliative and family medicine patients.
Providers are assigned specific zip codes in Harris County and are supported by case managers to address patient resources or supply needs. Each team has nursing support to assist with lab work and vaccines, as well as a triage nurse to answer any questions by phone. Operations coordinators support by processing new referrals or scheduling follow-up appointments.

YBC Foundation Donates $50K for Patients in Harris Health’s House Call Service
Harris Health's House Call Service received a $50,000 donation from the YBC Foundation to ensure low-income elderly and ill individuals who are in-between jobs can provide for themselves and their families.
The YBC Foundation was created by Younas Chaudhary and his wife, Bushra in 2004. “We believe that all human beings must be treated with dignity, respect, care and love," Younas says. “Our vision is to help the elderly, the sick, single parents, those who are temporarily unemployed, individuals who face religious persecution in their home countries or need financial assistance during challenging times."
The donation came as a surprise to the house call team and is greatly appreciated.
“We are thankful to the foundation and Mr. and Mrs. Chaudhary," says Christy Chukwu, senior operations manager, Population Health. “With this generous gift we're going to be able to help so many patients."
The team will purchase items that aren't covered by insurance, such as blood pressure machines, pill box dispensers, bed pads and waterproof mattress protectors.
