Harris Health established its Geriatric Medicine Program to address the specific age-related healthcare needs of seniors throughout Harris County. We provide seniors with access to exemplary and age-appropriate primary care, in-depth evaluations and leading-edge treatment.

Harris Health’s
Geriatric Clinic and the
Silver Seniors Clinic, located at
Baytown,
Casa de Amigos and
Settegast health centers, provide seniors with an array of convenient services. The team works with each patient and the family and/or caregiver to develop and individualized plan of care. Harris Health also offers care for homebound patients through its House Call Service. Physicians and nursing staff routinely visit with homebound patients, which allows patients to continue living in their home environment for as long as possible.
Every geriatric services staff member has received special, intensive training in the care of the elderly. What’s more, research shows geriatric patients have better outcomes with an interdisciplinary team approach in which physicians, nurse practitioners, nursing staff, physical and/or occupational therapists, dietitians, pharmacists, recreational therapists, case managers and social workers evaluate the patient to determine the best course of treatment.
Special emphasis is given to evaluating and improving patients functional abilities. Social workers routinely work with patients and their families to access community resources that allow patients to live as independently as possible for as long as possible.
