Livongo Pre-Diabetes Prevention Program

The program is offered at no cost to:
  • Active employees who are enrolled in a Harris Health medical plan and meet the program eligibility criteria
  • Spouses who are enrolled in a Harris Health medical plan and meet the program eligibility criteria
  • Retirees under the age of 65 who are enrolled in a Harris Health medical plan and meet the program eligibility criteria
Please submit an updated wellness screening form to Cigna to determine your eligibility for the program.
