Harris Health System interviewed hundreds of patients to assess the barriers that make it difficult for so many to live a healthy life. What we’ve learned from research, and was confirmed by our own patients, is that conditions in places where people live, learn, work and play can be far more influential to a person’s health than medical care. We’ve also learned that social determinants of health can vary from place to place.
That is why Harris Health is leading the way in our community by investing in resources and partnerships to better understand and address these health-related social needs. We’re using what we’ve learned from our patients to design and implement a transformative model of care, one that cultivates nurturing relationships with our patients, and partners with community organizations to improve our patients’ total well-being.
We are providing meaningful solutions and support to help our patients achieve healthier outcomes and a better quality of life.
