Harris Health’s Geriatric Clinic offers a holistic approach to caring for older adults over the age of 60. The team works with each patient and their family and/or caregiver to develop an individualized plan of care, providing comprehensive geriatric care to older patients to help them attain a comfortable quality of life in their later years.
The clinic’s team of professionals are sensitive to the special healthcare needs of older adults. They serve patients who experience:
- Memory problems
- Functional deficits
- Complex social situations
- Depression
- Abuse and neglect
- Difficulty maintaining independent living situations
- Any geriatric syndrome including frequent falls, incontinence, osteoporosis, malnutrition and hearing loss
Each new patient is seen and evaluated by a board-certified geriatrician or a board-certified geriatric nurse practitioner and a geriatric social worker. The geriatric team works together to determine a treatment plan that meets the patient’s individual needs.
