Silver Seniors Clinic

Harris Health System's Silver Seniors Clinics provide primary care to mature adults. Each clinic has a specialized team that works with each patient and the family and/or caregiver to develop an individualized plan of care.

Harris Health aims to help seniors with their health and way of life, preventing illness and treating disease and disabilities that can happen later in life.

The Silver Seniors team members include:

• Board-Certified Geriatricians (doctors)
• Nurse Practitioners
• Social Workers
• Nurse Case Managers
• Primary and Specialty Care Nurses and Certified Nurses Assistants
• Physical and Occupational Therapists

The Silver Seniors Clinics serve people ages 60 and older with one or more medical problems, or people of any age who have: 
• Memory problems
• Problems doing everyday tasks
• Depression
• Abuse and neglect
• Trouble living alone without help
• Problems such as frequent falls, loss of urine or bowel control, osteoporosis, poor nutrition or hearing loss
What Sets Silver Seniors Clinics Apart?
Harris Health’s Silver Seniors Clinics include the patient’s family and/or caregiver because having the support of a loved one can mean better overall health.

Patients are seen by a nurse case manager, social worker and a doctor or nurse practitioner who work together to make a treatment plan that meets the patients' needs.

Silver Seniors Clinics offer:
• House Call Program
• Skilled Nursing or Rehabilitation Care
• Inpatient Care
• Palliative Care (pain and symptom control)
• Adult Protective Service Evaluations
• On-site Lab and X-ray
• Physical Therapy and Occupational Therapy
